What Breast Augmentation Can Accomplish
Breast augmentation involves the placement of implants to increase the size of breasts that are naturally small or have lost volume after having children and nursing. Breast implants can also correct asymmetry between the breasts. By boosting their breast size, some women look more proportional. They often discover it is easier to shop for clothing (bathing suits in particular) that fits and flatters their bodies. Making Decisions about Breast Implants and Their Placement
Once a patient has decided to move forward with breast augmentation, she has some decisions to make. At the time of consultation, Dr. Green discusses the procedure and the choice of implants in detail. He works with each patient to help her select the appropriate size and type of implant, and solidify details about incision and implant placement. The alternatives are discussed, including the use of fat injection, avoiding the use of implants.
Implant Size: This is a very personal choice, as different patients have different goals. Usually, patients don’t know exactly how much volume they would like to add to their natural breast size. During the initial consultation Dr. Green uses a very precise sizing technique that allows you to accurately choose optimal implant size. Also, special sizing implants are used at the time of surgery to assess the appearance of the desired volume, and slight modification may be necessary to achieve the optimal result. A range of breast sizes is agreed on before the procedure is undertaken.
Implant Type: Dr. Green prefers the smooth round cohesive gel implants. These currently have the best track record for longevity and the lowest rupture rates. Smooth implants remain unattached to the surrounding tissue and do not have the problem of tethering and puckering that the textured implants sometimes have. Smooth implants have to be round since they are somewhat mobile and a specifically shaped implant is unlikely to stay in a particular orientation. There are new highly cohesive implants now that avoid problems with visible wrinkling.
Incision Location: Dr. Green prefers the transaxillary approach, which means that the incision is placed in the armpit (axilla), because, among other reasons, any scarring is very inconspicuous once the patient has fully healed, and there is no scar on the chest at all. He has been using this approach for several years and this has now become the first choice.
Dr. Green uses the Keller Funnel, which allows him to place a cohesive gel implant through the axilla. In contrast to other surgeons, Dr. Green has found that there are no increased risks with this approach and he can very easily place the implant exactly where he wants. Under certain circumstances, other approaches may be preferable; for example, if breast augmentation is combined with breast lift, the incision pattern may differ.
Implant Location: Dr. Green prefers sub pectoral implant placement, especially in women with very little breast tissue, because it provides more coverage of the implant, and because with this placement location the augmented breasts are less likely to appear or feel unnatural. Sometimes, however, it may be advantageous to place the implant directly behind the breast tissue; providing there is sufficient breast tissue to camouflage the implant. In weightlifters Dr. Green prefers to place the implants under the muscle.
Breast augmentation is performed on an outpatient basis and typically takes two hours. Anesthesia is administered prior to starting the operation.
Dr. Green makes the surgical incisions and places the implants according to the pre-determined surgical plan. The preferred incision is in the armpit(axilla). When he is finished with the procedure, he closes the incisions with sutures. A post-operative garment is placed around the breasts to apply gentle pressure as the tissues heal.
Recovering from Breast Augmentation
Patients must have a companion drive them home and help them get situated in a comfortable position for recovery. Right after surgery, bruising, swelling and breast tenderness are normal side effects; these effects should mostly subside within about two weeks.

Patients should rest as much as possible during their initial recovery. Bending, lifting and straining must be avoided. Dr. Green clears patients to return to work, exercise, socializing and other activities as he monitors their recovery.
Dr. Green likes to schedule an in-office follow-up visit with his breast augmentation patients two to three days after surgery. This appointment is a good opportunity to catch any (rare) problems early and prevent complications. The next follow-up appointment is about a week later, at which point the stitches are removed. Further follow-up will be arranged as necessary over the next weeks and months. Usually Dr. Green will see his patients annually or as needed. There is no charge for post-op office visits
